RN - Cardiac Rehab - Per Diem

Job Responsibilities:

Job Summary:               
Provides direct patient care in Out-Patient Cardiac/Pulmonary Rehabilitation. Monitors patients during exercise sessions. Nursing responsibilities focus on preventative and restorative healthcare needs of the cardiovascular patient. Provides patient education on disease entities, cardiac and pulmonary risk factors, and behavior modification.
 
Accurate and Timely Patient Care
• Assists in and accompanies patients in the admission, transfer and discharge procedures.
• Provides patient with accurate and timely personal care, taking and recording vital signs and taking and recording temperatures, pulse, respiration, weight, height, blood pressure, pulse oximetry and intake/output measurements as delegated by Staff Registered Nurse.
• Performs phlebotomy on patient
• Performs EKG
• Places patient on monitor, telemetry, and pulse oximeter.
• Completes personal care needs for patients including bathing, feeding, dressing, toileting and ambulating
• Assists patients with handling and care of clothing and other personal property.
• Transports patients to and from various departments as requested
• Documentation meets current standards and policies of organization and unit.
• Follows the plan of care established by RN

Environment Free from Contaminants & Hazards

• Assists in maintaining a safe and clean environment by cleaning and stocking the utility/equipment rooms.
• Reports equipment that is out of order.
• Supports a culture of safety and quality patient care.  Communicates appropriately and clearly to Registered Nurse colleagues, co-workers and physicians
• Manages and operates equipment safely and correctly.
• Appropriately handles clean and dirty supplies on the unit and reports other problems to Registered Nurse.
• Effective Collaboration that Helps Achieve Organizational Goals
• Participates in department and/or system wide team dynamic initiatives.
• Communicates effectively to optimize patient care, expresses positive customer relations and contributes to a supportive and cooperative working environment.
• Provides day-to-day support of workflow within the work unit.
• Interacts professionally with patient/family and involves patient/family.
• Acts as a team player and works collaboratively within the team and organization.

Useful Suggestions for Process Improvement

• Identifies better, faster, more efficient ways of doing tasks as well as eliminates unnecessary work and all non-value-added activities to enable a focus on work that generates greater value.
• Responds to requests from Clinical Supervisor, and collaborates with interdisciplinary teams to assist with evaluation of programs and services.  Communicates with Clinical Supervisor regarding cost savings initiatives and planning.

 


Job Qualifications:

Education & Experience
Registered Nurse

Licenses and/or Certifications
•Must have current ACLS Certification

Skills
•Demonstrates an ability to be flexible, organized and function under stressful situations.
•Knowledge of medical terminology preferred.
•Must possess ability to successfully interact with stakeholder (patients, providers, families, staff, management, physicians and general public.)

Language Ability
•Ability to read, write and interpret documents in English such as safety rules, operating and maintenance instructions, and procedure manuals. 

Math Ability
•Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.  Ability to calculate figures and amounts such as proportions, percentages, rates and ratios.

Reasoning Ability
•Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 

Physical Demands
•Walking, sitting, repetitive motion, lifting/pushing/moving minimum of 50 lbs, has the ability to stand for long periods of time i.e. 50 minutes.  Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions of the position without compromising patient care. 

Working Conditions
•Potential for exposure to chemicals and communicable diseases and related medical waste.
•Supervisor regarding cost savings initiatives and planning.
