Job Summary:           

The Advanced Practice Provider is a Nurse Practitioner (NP) that works in a collaborative relationship with a physician provider to oversee the care delivered to a patient population, and meet Hospital goals for safety and quality. The NP is responsible for direct patient care, documentation of care delivered, adherence to hospital and nursing policies, procedures and practice standards, education of patients and staff, assurance of service excellence, completion of state and hospital-required reviews of practice, and participation in performance improvement and other activities as required.

 

Job Results & Essential Functions

 Patient & Family Centered Care

  1. Ability to formulate an individualized plan of care, revise plan as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness.  Coordinates the nursing plan of care with the medical plan of care.

  2. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.

  3. Demonstrates expertise in the care of acute care patients providing collaboration, direction and instruction to the medical services staff in the application and utilization of the nursing process.

  4. Incorporates principles, evidenced-based practices and research that promote, maintain and restore the health of patients with specific healthcare issues across the life span.

  5. Is a mentor to nursing staff regarding clinical, professional and research practices.

  6. Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills.

  7. Treats patients and their families with respect and dignity.  Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.  Functions as liaison between administration, patients, physicians and other healthcare providers.

  8. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served.  Able to interpret data about the patient’s status in order to identify each patient’s age specific needs and provide care needed by the patient group.

  9. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

 

Team that Consistently Delivers

  1. Provides day-to-day direction to staff and coordinates workflow within the work unit; including distributing work, disseminating information, assigning staff to cover for absences, providing technical assistance, answering questions and resolving problems.  Ensures work is scheduled effectively, and checkpoints and controls are set for monitoring.
  2. Assists the manager with orientation, supervision and evaluation of clinical staff.  Ensures new hires are knowledgeable of policies, work rules, job responsibilities and other information necessary for successful performance.
  3. Assists with the establishment of standards of performance and maintains an evaluation system for clinical personnel.
  4. Ensures an affective learning environment by coaching employees, orchestrating learning opportunities, and providing relevant, high-impact feedback. 
  5. Provides employees with current, direct complete and “actionable” positive and corrective feedback.  Documents employee performance issues.
  6. Addresses conflict within the team and facilitates an effective resolution.

 

Process Improvement

  1. Identifies better, faster, more efficient ways of doing tasks as well as eliminates unnecessary work and all non-value-added activities to enable a focus on work that generates greater value.

  2. Responds to requests from Clinical Supervisor, and collaborates with interdisciplinary teams to assist with evaluation of programs and services.  Communicates with Supervisor regarding cost savings initiatives and planning.

  3. Utilizes understanding of regulatory requirements to identify ways to improve process quality and compliance.

  4. Participates in performance improvement activities; mentors staff regarding the performance improvement process; shares results of performance improvement activities with staff.

 

 

Position Qualifications

 

Education & Experience

Graduate of an accredited school of nursing plus two (2) years of related clinical experience as a Nurse Practitioner.

 

Licenses and/or Certifications

  • National Certification

  • Current New York State license as an Advanced Practice Provider.

  • Current CPR and BLS certification

  • Current ACLS/PALS/NRC/PEARS

     

    Skills

  • Extensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology and management of acute and chronic patient/family problems.

  • Substantial interpersonal skills necessary to instruct patients and their families and to collaborate with health team members.

  • High level of analytic ability to concentrate and pay attention to details.

 

Physical Demands:   Ability to move or transfer patients with an individual effort equal to 50 lbs.  Nurses should have a full range of body motion to successfully perform all essential functions of the job, meaning you’ll have to stand, walk, bend, kneel, crouch and reach frequently during shift.

 

Mental Demands:    Ability to function during stressful medical emergencies calmly and efficiently. Ability to plan and revise care of patients according to their needs.

 

Interactions:   Internal and external customers, Physicians and co-workers.

 

Working Conditions:     Potential for exposure to infectious pathogens, physical hazards, cold or heated conditions. Personal protective equipment is available to all team members. Hours of work may vary, on call is required.

 

Working Conditions

Works in normal patient care environment where there is minimal physical discomfort due to temperature, noise or dust.  May encounter exposure to chemical toxins, human blood and tissue, crowed work areas,

Electrical equipment, communicable diseases and related medical waste.  Potential exposure to chemicals, communicable diseases and related medical waste.

Job Qualifications:
Express Care (Walk-In Clinic):
Per Diem positon available, operates 10 am - 8 pm. Provide care for patients in an express care level (Walk in Clinic). Work collaboratively with Emergency Physician and nursing staff. Patients require evaluation, intervention, and disposition. Excellent communication skills and teamwork required. Must be able to function as a solo provider.
