SUMMARY OF POSITION:
ACCESS Family Care is a federally qualified health center providing comprehensive, patient-centered primary care services to a diverse population including individuals and families who are low income, multi-ethnic, uninsured and underinsured. We provide these services at multiple locations. The Behavioral Health Consultant works under the direction of the Health Home Director, with collaboration from the Chief Medical Officer as needed. The Behavioral Health Consultant will work with a Health Care Team to coordinate case management and patient follow-up. The Behavioral Health Consultant will work with local substance abuse treatment programs in appropriate transfer of care and follow-up services of care for patients, providing individual and/or group sessions for patients with substance abuse and mental health issues.
REQUIRED SKILLS AND ABILITIES
Prefer LCSW or Clinical Psychologist. Bilingual skills are a plus.
