Job Description/Essential Functions:
District 19 Community Services Board Developmental Disability Services, located in Petersburg, VA is offering a PRN Case Management opportunity. This position will perform documentation, case management, service coordination, screening and/or assessments for children and adults diagnosed with developmental disabilities as needed. The Developmental Disabilities Case Manager (PRN) is responsible for performing those specific duties identified as needed to support the full time CM team with Waiver and SPO Case management services in accordance with the local, state and federal quality assurance standards. The PRN case manager shall be assigned specific duties assigned by the Manager or Director to assist the CM team with coordinating the delivery of services for consumers and their families.  They may be tasked with the development of Person-Centered Service Plan, completion of quarterly reports and assessments, and any other CM related responsibilities that may assist with the filling in of currently identified gaps of services.

Requirements: (Experience, Essential Education, and Skills Needed):
Must meet the DBHDS definition of "Qualified Developmental Disabilities Professional” (QDDP/QDIP/QMRP) 
Must possess at least one year of documented experience working directly with individuals who have mental intellectual/developmental disabilities and one of the following credentials: 
1. a doctor of medicine or osteopathy licensed in Virginia, 
2. registered nurse licensed in Virginia, or 
3. completion of at least a bachelor's degree in a human services field, including, but not limited to sociology, social work, special education, rehabilitation counseling, or psychology. 

*Must have a valid Virginia Driver’s License 

*Must have basic computer skills 
A bilingual (English/Spanish) candidate is preferred.

All D19 employees that provide case management services must complete the case management curriculum developed by DBHDS within 30 days of employment. Developmental case managers must complete the ISP training modules developed by DBHDS within 60 days of hire.

Rate:
Position is a grade 208, starting rate is $24.60/hour. 

Salaries are negotiable. 

Hours: 
Monday - Sunday, to include evenings and weekends as needed.

Shift and Schedule:

Flexible work around your schedule (days, evenings or weekends)

Hours:

Work 5-28 hours per week.

Telework: 
This position is eligible for some telework at the discretion of the supervisor and in accordance with the telework policy.

Benefits:
This position works on an as needed basis (PRN), not benefits eligible.  

Position requires valid Driver’s License in state of residence with a point balance of -6 or higher, and an extensive pre-employment background check and drug screening.
