Job Description/Essential Functions:
District 19 Community Services Board is seeking an experienced Master’s level clinician.

Serves as liaison between D19 and state hospitals for patients who have been civilly admitted to those facilities. Provides advanced level of inpatient hospital management and person-centered discharge planning services. Services are to include assessment of patient's needs, referral to community housing resources, referral to Region IV programming, utilization of state hospital discharge protocols, and ability to access state (DAP) and local (Region IV/other) funding streams to maximize patient access to financial resources. 

Forge and maintain relationships with other CSBs and community stakeholders, transfer and accept cases from other CSB’s and accept consumer from other State facilities.
 
Certified Preadmission Screening Clinicians - this position conducts pre-screenings and recommitment evaluations, representation at Involuntary Commitment hearings. Position includes conducting prescreening evaluations in the community, visiting consumers in hospital and community settings when needed.





Requirements: (Experience, Essential Education, and Skills Needed):
Successful applicant must have:

Master’s degree in human services (which includes social work, gerontology, psychology, psychiatric rehabilitation, special education, sociology, counseling, vocational rehabilitation, and human services counseling).

LMHP-R or licensed as an LPC, LMFT OR LCSW in Virginia. 

Mental health treatment experience and Emergency Services experience.

Requirements to obtain Certification as a Pre-Admission Screening Evaluator within four weeks of hire, experience in working with seriously mentally ill in a mental health setting and patient service planning.

Knowledge of the Virginia Code as it relates to temporary detentions and involuntary commitments, of assessment and diagnosis, and of substance abuse, certification eligibility as a crisis pre-screener. 

Experience working with SMI population providing case management outreach, basic computer skills and ability to work independently with minimal supervision.

Essential job duties include assessing the needs of consumers with intense needs that have been admitted for psychiatric treatment at
state and private hospitals. Assist with clinical oversight of consumers in State hospitals, participate in treatment teams, discharger planning,
secure guardians and payees for consumers when needed and determine the most appropriate discharge setting and funding streams. Locate resources for consumers upon hospital discharge.  

This position requires travel to multiple sites located throughout the District 19 catchment area and out of the catchment area.

Salary:
Position is a grade 211, starting salary is $53,322.00.
Salaries are negotiable.
