District 19 CSB is offering 2 rewarding full-time School Based Mental Health Case Manager positions to provide a variety of case management services to children and adolescents located in Petersburg, VA. Essential job duties of this position include:  assessment and planning services to include developing an ISP per policies and regulations; monthly contacts, activities or communication relevant to the ISP; coordinating services and treatment planning with school personnel and providers; monitoring and assisting with accessing medical/psychiatric services; providing crisis and behavioral interventions.

The incumbent in this grant-funded position will perform case management duties with youth who are identified by the school and are eligible for the service.  Case Management means- services that can include assistance to individuals and their family members in accessing needed services that are responsive to the person's individual needs.

Case managers are hired with the goal of becoming welcoming, recovery-oriented, and co-occurring competent to engage all individuals receiving services in empathetic, hopeful, integrated relationships to help them address multiple issues successfully.

Successful applicant must have: A minimum of a Bachelor’s degree in Human Services; experience in clinical record documentation and meet QMHCM qualifications per Virginia regulations; Proficiency with personal computers and at least one year of experience with children and adolescents.
