Job Description/Essential Functions:
District 19 Community Services Board Developmental Disability Services, located in Petersburg, VA is offering a full time Case Management opportunity. This position will perform case management, service coordination, screening and assessments for children and adults diagnosed with developmental disabilities.

The Developmental Disabilities Case Manager is responsible for the overall assessment of consumer’s strengths and limitations and the initiation of Waiver and SPO Case management services in accordance with the local, state and federal quality assurance standards for case management.  The case manager shall coordinate the delivery of services for consumers and their families assigned to his/her caseload by the Manager.  This includes (but not limited to) the development of Person-Centered Service Plan and assist eligible consumers in accessing to the most appropriate services available to meet their individual needs in the least restrictive environment.

Developmental Disabilities Case Manager is required to perform activities designed to assist an individual with DD in accessing needed medical, psychiatric, social, educational, vocational, residential, and other supports essential for living in the community and in developing his or her desired lifestyle.

Requirements: (Experience, Essential Education, and Skills Needed):

Must have completed a bachelor's degree in a human services field, including, but not limited to sociology, social work, special education, rehabilitation counseling, or psychology.

Must have at least one year of documented experience working directly with individuals who have developmental disabilities.

Home visits required, driving agency vehicle to conduct agency business, sitting at a desk for long periods of time, ability to make sound clinical judgment.

A bilingual (English, Spanish) candidate is preferred. 

*Must have a valid Virginia Driver’s License.

Salary:
This is a Grade 208, starting salary is $44,771.00.
Salaries are negotiable.

Telework: 
This position is eligible for some telework at the discretion of the supervisor and in accordance with the telework policy.

PSLF is a federal program available to full-time government and nonprofit employees who have Federal Direct Loans and have made 120 payments on the Federal Direct Loans (after Oct. 1, 2007) may be eligible for the remaining loan balance to be forgiven. Position requires valid Driver’s License in state of residence with a point balance of -6 or higher, and an extensive pre-employment background check and drug screening.
