Job Description/Essential Functions:

District 19 CSB offers a challenging full time Case Manager position to provide case management to children and adolescents located in all nine (9) D19 service areas.  Incumbent serves as primary case manager/service coordinator for children who receive Child Mental Health Initiative (MHI) and CSA funding for ICC services. Serves as primary provider/CSA Family Assessment and Planning Team representative for case management services to the SED and At-Risk children and adolescents in need of residential and community-based care. Position works closely with other staff in using a team approach to enhance psychiatric stability, quality of life, access to available resources appropriate use of medical services and inpatient care. Responsibilities include assessment, coordination and monitoring of community services and supports, outreach and home visits, crisis prevention/intervention and advocacy.


Requirements: (Experience, Essential Education, and Skills Needed):

Successful applicant must have: Bachelor's degree in human services; Medicaid qualifications for Mental Health Case Manager (QMHCM). Knowledge of computers/EHR and clinical record documentation; experience in clinical services in a multicultural community setting; and valid VA driver’s license with acceptable driving record. Must meet CANSVA-certification requirements for Family Assessment and Planning case management responsibilities.

Prefer crisis intervention experience, knowledge of community resources, and Microsoft Office experience.




Preferred Qualifications / Other Relevant Job Information:

This is a Grade 208 salaried position.  The minimum salary is $44,771.  Salary is negotiable.
