POSITION SUMMARY

 

The In-Home Support Specialist provides the needed support and guidance to individuals with developmental disabilities. This involves implementing habilitation goals in all areas of activities of daily living (ADL’s) as directed by individual’s Plan of Care (POC). This position reports directly to the HCB Waiver Program Coordinator.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES

 

1.       Provide training to individuals with developmental disabilities in the areas of self-care, communication, self-direction, independent living, mobility and learning as directed by the Plan of Care (POC) in order to enhance each individual’s skill development, health, freedom, and personal dignity.

2.       Assists as needed with bathing, grooming, and dressing of individuals, monitoring their appearance to ensure that each person is dressed and groomed appropriately.

3.       Assist individuals during activities by conducting therapeutic activities, such as physical exercises, occupational arts/crafts and recreational games. Work on motor skill development as instructed by a therapist.

4.       Train and support individuals in daily tasks, such as cleaning of living areas, personal laundry responsibilities, meal preparation, and community outings. Works with individual on range of motion techniques if needed. May work with speech and language skill development, once training is received.

5.       Report the development of individual skills by observing and providing detailed documentation on progress to Care Coordinator.

6.       Assist with maintaining a clean and orderly home environment.

7.       Assists individual with interaction of peers or social events.

8.       When properly authorized and certified, supervise individuals in taking medications as prescribed by the physician.

9.       Adhere to confidentiality guidelines, as outlined by organization policy and HIPAA regulations.

10.    Attend and maintain all training requirements in accordance with current applicable Federal, State, and local requirements, as well as organizational policies and procedures.

11.    Provides required documentation to Program Coordinator on a bi-weekly basis.

12.    Serves as an advocate for the client and intervenes in crisis situations when appropriate.

Job Qualifications:

MINIMUM REQUIREMENTS

 

In-Home Support Specialist I (Range 11)

Minimum of 18 years of age with a High School diploma or G.E.D. Current valid Driver’s License and acceptable driving record preferred. Able to read, write, and speak the English language in an understandable manner. Ability to solve practical problems and deal with a variety of situations without supervision. Ability to interpret a variety of instructions furnished in written, oral or schedule form. Capable of providing proper communication and demonstration skills in order to provide the quality training necessary to achieve independent success. Have or be able to get certification in First Aid/CPR. Able to assist individuals with mobility issues. Must frequently life/move up to 10 pounds and occasionally lift/move more than 100 pounds.

 

In-Home Support Specialist II (Range 12)

Requirements of Specialist I: Certificate in direct Service Provider or related field and 1 year of work experience in the field of Developmental Disability Services or 3 years’ experience as an In-Home Support Specialist. An Associate’s Degree in Disability Services or related field may be substituted for Certificate in Direct Home Service Provider or related field and 1 year work experience in the field of Developmental Disability Services.

 

In-Home Support Specialist III (Range 13)

Requirements of Specialist I/II: Associate’s Degree in Disability Services or related field and 2 years’ experience in the field of Developmental Disability Services or 5 years’ experience as an In-Home Support Specialist. Bachelor’s Degree in Human Services or related field may be substituted for Associates degree in Disability Services and 2 years’ experience in the field of Developmental Disability Services.
