POSITION SUMMARY

Responsible for carrying out the daily habilitation goals and objectives for qualified HCB Waiver clients in community settings as assigned. This position reports directly to the Senior and Disabilities Services (SDS) Director.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES

1.       Treats individuals with respect and upholds their human rights.

2.       Provides direct client support during any scheduled involvement in community settings while encouraging maximum independence, safety, and healthy habits while out in the community.

3.       Provides individualized skills training related set Plan of Care goals and objectives that fosters successful participation in community settings.

4.       Participates in the design of and on-going implementation of individualized service plans for all qualified clients receiving Day Habilitation Services while maximizing the use of community resources.

5.       Meets regularly with clients Care Coordinator for client goal and objective progress.

6.       Maintains good communication with clients’ guardians of care providers, ensuring any important need to know information is shared.

7.       Submits progress reports on a regular basis.

8.       Assists in any reports pertaining to Day Habilitation services.

9.       Assists in the arrangements for individual transportation and times to community setting events pertaining to individual goals and objectives.

10.   Completes all required documentation and assists with any needed amendments to individual service plans.

11.   Assists with internal and external audits and evaluation needs.

12.   Complies with Maniilaq SDS program standards, and Medicaid requirements.

13.   Maintains the required State licensing, continuing education hours annually.

14.   Maintains good driving record for continued Maniilaq insurance coverage.

15.   Maintains the ability to be flexible with scheduling Day Habilitation service coverage, including possible special community events.

16.   Maintains First Aid/CPR certification.

Job Qualifications:

MINIMUM REQUIREMENTS

High school diploma or equivalent preferred. The desire to work with people with developmental disabilities. Ability to lift up to 75 pounds and assist clients with mobility issues. Must obtain CPR/First Aide certification within 6 months of hire. Ability to accurately and efficiently use standard office computer equipment and software, faxes, copiers, telephones, and 2-way radios. Alaska Driver’s License required with a good enough record to be insured to drive Maniilaq vehicles.
