POSITION SUMMARY
This position carries out and assists in the delivery of nursing services to residents, including oversight and direction for caregivers. Individual is responsible for upkeep of all resident medical records and care related data. Individual works directly under the Director of Nursing or designee.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Initiates and utilizes the nursing process to meet the holistic needs of the patient and family while adhering to the professional standards of conduct, ethics, and practice.
1. Observes the resident’s clinical condition: recognizes, identifies, and reports serious and/or emergent situations.
2. Integrates evidence based practices when developing plan of care and interventions.
3. Assists in patient/family teaching programs.
4. Evaluates the residents’ plan of care and revises plan as needed.
5. Safeguards the resident’s right to privacy by judiciously protecting information in a confidential manner.
6. Understands and practices facility safety and emergency preparedness procedures.
7. Assumes responsibility and accountability for individual nursing judgments and actions.
8. Assumes charge nurse duties as assigned.
9. Participates in departmental and organizational performance improvement activities.
10. Utilizes incident and medication error reporting protocols per policies.
11. Monitors, evaluates and responds to CNA observations and documentation of residents’ behaviors, physical condition, and abilities.
12. Recognizes respects and promotes the integration traditional native values and customs into daily care and routines of the resident.
13. Manages own work assignments, provides feedback, and redirects services as directed by Resident Care Manager or Director of Nursing Services.
14. Prepares or assists with new admissions, transfers, and discharges, providing direct resident care as determined by resident condition.
15. Completes treatments and procedures, within scope of practice and related training.
16. Evaluates and records residents’ needs and responses to medications and administers medications as ordered by the physician and directs medication aides accordingly.
17. Monitors resident skin condition and reports changes or new problems to physician and Resident Care Manager and monitors residents on alert status.
18. Completes incident forms, transcribes and carries out physician orders and notifies family and physician as directed.
Minimum Requirements
Must have a current, unencumbered license as Licensed Practical Nurse or Licensed Vocational Nurse. Excellent written and oral communication and motivational skills are essential to success. Ability to lead and supervise paraprofessional staff. Must be able to receive direction from supervisor, adapt to changing situations and multitask in daily execution of duties. Must be able to perform the essential job duties with or without reasonable accommodations. Able to lift 50 pounds.
