POSITION SUMMARY

Through a collaborative process, the Case Management Support/Outreach Specialist will facilitate efficient Diabetes Care coordination. They will be able to utilize effective communication and available resources, to promote patient safety, quality of care, positive outcomes, and patient satisfaction.  They will also be responsible for engaging in and developing culturally appropriate events, with a particular focus on subsistence activities, which promote healthy living. This job requires village travel and formation of partnerships with tribal councils and other community leaders. Reports directly to the Diabetes Program Manager.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES

 

  1. Carries out reception duties including receiving, screening, and forwarding incoming phone calls, and prioritizing and passing on information.
  2. Performs clerical duties as required to support the Diabetes program, including typing letters, memos, reports, and faxing/copying. Maintains files in a secure and organized manner.
  3. Maintains the EMR Referral Management Queue through daily monitoring, scheduling, and follow-up.
  4. Maintains spreadsheets used to track Diabetes Key Measures.
  5. Coordinates and makes travel arrangements for clients traveling to the Diabetes Specialty Field Clinics in collaboration with the Maniilaq Specialty and Travel departments.
  6. Participates in statewide diabetes prevention activities including travel to the villages within the Maniilaq service area.
  7. Participates in community awareness events that highlight wellness through increased physical exercise, making healthy food choices, and tobacco cessation.
  8. Responsible for assisting patients with questions, diabetes related appointments, and assists with medication refills.
  9. Performs technical aspects of filling prescriptions such as collecting patient demographics, processing refills, counting/measuring and/or reconstituting drug products, compounding medications when needed and making sure medications are delivered in a timely manner.
  10. Supports and participates in teaching and training to expand services and increase public awareness of diabetes prevention and control.
  11. Attends team conferences
  12. Maintains professional ethical standards of conduct at all times.
  13. Other duties as assigned
Job Qualifications:

MINIMUM REQUIREMENTS

High School Diploma or equivalent required. Six months experience in case management required.  Must have a valid Alaska Pharmacy Technician License. Ability to utilize various computer programs, including Cerner, Microsoft Office, PharmNet Retail Manager, and ScriptPro. Ability to interact in an effective and respectful manner with health team members, community agencies, and clients/families.  Ability to work autonomously with little direction and self-motivated.
