POSITION SUMMARY

This position is responsible for developing the Long TermCare activity program through development and implementation of an appropriateand culturally sound activities program. Facilitates activities with theresidents and documents participation. This position reports to theAdministrator of Long Term Care.

                                               

PRINCIPAL DUTIES AND RESPONSIBILITIES

1.                  Formulates and implements activities which are bothstimulating and culturally appropriate for the Elders. This may include outingsto Lavonne’s fish camp, driving to go berry picking, going ice fishing, goingto the park service, etc.,

2.                  As member of IDT team, responsible for working withother team members to develop appropriate plans of care, doing assessments andwriting CAAs.

3.                  Write progress notes on a monthly basis for eachresident.

4.                  Periodically review activities program effectivenessand participation and make changes accordingly.

5.                  Conducts/Assists with activity programming per theresidents’ plan of care.

6.                  Leads monthly resident meeting and acts as a liaison tothe Administrator for issues that arise. Forwards concerns/grievances toappropriate departments and shares those responses with the Elders.

7.                  Maintains confidentiality in regard to patient careinformation and records at all times.

8.                  Provides written reports of activities to theAdministrator as directed.

9.                  Oversees program activities for the Elders.

10.               Responsible for keeping policies current and programcompliant with CMS rules and regulations.

11.               Participate in the facility’s QAPI committee.

 

Job Qualifications:

MINIMUM REQUIREMENTS

Must have a high school diploma or equivalent. Must be aqualified therapeutic recreation specialist or an activities professional whois: A) licensed or registered if applicable by the state in which practicingand is eligible for certification as a therapeutic recreation specialist or asan activities professional by a recognized accrediting body on or after October1, 1990 or B) has 2 years of experience in a social or recreational programwithin the last 5 years, one of which was full time in a patient activitiesprogram in a health care setting or C) is a qualified occupational oroccupational therapy assistant or D) has completed a training course approvedby the state. Computer literacy and direct experience in elder servicespreferred. Familiar with the Northwest Region, people and culture preferred.Knowledge of cultural and social conditions within the geographic area in whichservices are provided, and how these conditions will effect services preferred.
