POSITION SUMMARY
Responsible for the Care Coordination for all recipients of IDD, CCMC, and APDD Medicaid Waiver Services in our region. Oversees the Assistant DD Case Manager. Ensures the continuum of services provided to developmentally disabled individuals and their families in the Northwest Arctic Borough and Point Hope. This position reports directly to the Senior and Disabilities Services Director.
PRINCIPAL DUTIES AND RESPONSBILITIES
MINIMUM REQUIREMENTS
Have 2 yrs of post-secondary education in a human services field and 2 yrs working in the field of developmental disabilities: or 5 years’ experience working with the individual with developmental disabilities. Have a minimum of 2 years of supervisory experience. Obtain certification as a Care Coordinator for the IDD, APDD and CCMC Medicaid Waiver Services Program. CPR/1st Aid certification. Ability to lift 40 lbs. Ability to accurately and efficiently use standard office computer equipment and software, faxes, copiers, telephones, and 2-way radios. Current Alaska Driver’s License with a good enough driving record to be covered by Maniilaq’s insurance. A willingness to fly on small aircrafts for village outreach travel required. Familiarity with the Northwest Arctic Borough, the culture, its people and the ability to speak Inupiaq is desirable.
