POSITION SUMMARY
This position is responsible for developing the Long Term Care activity program through development and implementation of an appropriate and culturally sound activities program. Facilitates activities with the residents and documents participation. This position reports to the Administrator of Long Term Care or designee.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Formulates and implements activities which are both stimulating and culturally appropriate for the Elders. This may include outings to Lavonne’s fish camp, driving to go berry picking, going ice fishing, going to the park service, etc.,
As member of IDT team, responsible for working with other team members to develop appropriate plans of care, doing assessments and writing CAAs.
Write progress notes on a monthly basis for each resident.
Periodically review activities program effectiveness and participation and make changes accordingly.
Conducts/Assists with activity programming per the residents’ plan of care.
Leads monthly resident meeting and acts as a liaison to the Administrator for issues that arise. Forwards concerns/grievances to appropriate departments and shares those responses with the Elders.
Maintains confidentiality in regard to patient care information and records at all times.
Provides written reports of activities to the Administrator as directed.
Oversees program activities for the Elders.
Responsible for keeping policies current and program compliant with CMS rules and regulations.
Participate in the facility’s QAPI committee.
MINIMUM REQUIREMENTS
Must have a high school diploma or equivalent. Must be a qualified therapeutic recreation specialist or an activities professional who is: A) licensed or registered if applicable by the state in which practicing and is eligible for certification as a therapeutic recreation specialist or as an activities professional by a recognized accrediting body on or after October 1, 1990 or B) has 2 years of experience in a social or recreational program within the last 5 years, one of which was full time in a patient activities program in a health care setting or C) is a qualified occupational or occupational therapy assistant or D) has completed a training course approved by the state. Computer literacy and direct experience in elder services preferred. Familiar with the Northwest Region, people and culture preferred. Knowledge of cultural and social conditions within the geographic area in which services are provided, and how these conditions will effect services preferred.
