POSITION SUMMARY

Under the direction of the Maniilaq Association Cancer program, the Tobacco Health Systems Change Coordinator will work to provide services to address prevention of tobacco, promote tobacco cessation services to tobacco users, eliminate exposure to secondhand smoke, train and support staff to make tobacco cessation a practice and system priority for the Maniilaq Association  Health System for the whole Maniilaq service area.  This position reports directly to the Cancer Clinical Nurse Manager or designee.


PRINCIPAL DUTIES AND RESPONSIBILITIES

  1. Assists with grant writing to solicit State of Alaska Department of Health (DOH) Request for Grant Proposal for Tobacco Prevention.

  2. Provides grant writing assistance in applying for continuation for Tobacco Prevention Grant and other funding sources.

  3. Monitors budget expenses for Tobacco Prevention services.

  4. Supports implementation of tobacco prevention, tobacco cessation, substance use intervention, and chronic disease prevention initiatives.

  5. Leads development and implementation of health systems change activities to improve screening, referral, treatment engagement, and continuity of care.

  6. Collaborates with Maniilaq Health Center departments to assess workflows and improve patient access to prevention and treatment services.

  7. Supports development and implementation of standardized screening protocols and referral pathways.

  8. Assists with optimization of EHR workflows, documentation standards, referral tracking, and clinical decision supports for Tobacco Prevention and Cessation.

  9. Coordinates with Quality Assurance and leadership to ensure staff meet orientation, training, documentation, licensure, and competency requirements around Tobacco Prevention and Cessation.

  10. Provides education, mentorship, and consultation to staff regarding prevention, behavioral health interventions, and evidence-based practices for Tobacco Prevention and Cessation.

  11. Participates in staffing, supervision activities, in-service training, and professional development.

  12.  Creates and reviews the referral process and assists in directing individuals to appropriate services to ensure continuity of care.

  13. Attends any State DOH-required meetings/conferences to maintain grant requirements.

  14. Orders all the necessary supplies, equipment, teaching aids, and materials to sustain programs.

  15. Submits quarterly reports to the DOH GEMS for Tobacco Prevention.

  16. Submits bi-monthly reports to the supervisor for Maniilaq Association Board meetings for Tobacco Prevention.

  17. Provides promotion of the Alaska Quitline throughout the service area.

  18. Works toward planning, development, and implementation of community awareness events focusing on the effects of tobacco use, and distribution of Alaska Quit Line materials.

  19. Participates in State of Alaska, Department of Health Tobacco Prevention and Control Program activities.

  20. Submits the Annual Service Plan to the Chief Nursing Officer for Tobacco Prevention.

  21. Travels to village clinics to educate staff on Alaska Quit Line, assists staff with training in tobacco cessation and screening.  

  22. Complete the Tobacco Treatment Specialist Training to provide Maniilaq healthcare and public health staff with evidence-based skills to help patients overcome nicotine dependence.

Job Qualifications:

MINIMUM REQUIREMENTS

High School diploma or equivalent is required. Must have an Associate degree in a health care related field. Ability to work without close on-site supervision, and be self-motivated with a willingness to verbally communicate with large group of people. Must possess strong technical, administrative, organizational, and financial skills. Able to lift 40 lbs. and travel in small planes. Complete the Tobacco Treatment Specialist Training.
