POSITION SUMMARY

The Psychiatric Clinical Provider (PCP) is a mid-level provider responsible for delivering team-based, social medicine-oriented psychiatric and addictions care in Outpatient Services, Inpatient Services, Women’s Health, telemedicine, community and emergency room settings. This is a complex and dynamic role that involves collaboration with primary care and community behavioral health partners.  This position provides direct patient care to address a wide range of social and psychiatric needs. The PCP will work with supervisors and Behavioral Health Director to maintain clinical protocols, workflow, policies, and procedures for the integrated behavioral health program. This position reports to the Behavioral Health and Medical Director.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES

Clinical Care Delivery and Management:

  1. Prescribes and manages psychiatric medications for a large patient panel within the guidelines established by the Medical Director and consulting psychiatrist;
  2. Performs behavioral health and social needs assessments in Outpatient, Inpatient, Women’s Health, telemedicine, and emergency room settings;
  3. Delivers brief targeted interventions to address social determinants of health, psychosocial aspects of chronic and acute disease management, behavioral health, and substance abuse treatment needs;
  4. Applies behavioral principles to address modifiable lifestyle and health risk issues;
  5. Communicates effectively with the care team, including providers, nursing staff, patient advocates, CHA/Ps, behavioral health aides, and case managers;
  6. Documents treatment process and progress in EHR;
  7. Maintains confidentiality of patient records and treatment;
  8. Provides consultation and clinical education appropriate to scope,
  9. Works collaboratively with Behavioral Health Services Director, Medical Director, and others to maintain the scope, policies, protocols, and practice of integrated and psychiatric care;
  10. Collaborates with research and evaluation teams to fulfill grant reporting and data collection requirements
Job Qualifications:

MINIMUM REQUIREMENTS

Must be a graduate of a regionally accredited advance practice nursing or physician assistant program, and must hold a current license in any state as a Physician Assistant or Nurse Practitioner. Substantive experience in psychiatric and addictions care is required. Additional experience in public health, social medicine, or related field is preferred. Must demonstrate knowledge of child and adolescent growth and development, assessment and treatment of psychiatric disorders, and culturally appropriate care. Must be able to work within the framework of established practice guidelines, including standing orders, to determine which patient conditions can be treated independently and which must be referred. The PCL must work collaboratively with clinical staff, carry a large patient panel, and translate clinical experience into sound policies and protocols to contribute to the success of an integrated behavioral health program.
