Care Coordination Representative

Key Duties and Responsibilities:


Providesan extension of our exceptional patient experience to current and prospectivepatients as well as insurance company partners and other third parties.


Aminimum of a High School diploma; some college preferred


Preferably1-2 years of customer service experience in healthcare industry


  • Primary task is to answer multi-line phone system in a professional and courteous manner within minimum rings.
  • Communicate with patients, physicians, health care professionals, and community members in a knowledgeable, compassionate, and helpful manner.
  • Explain services provided by the Company.
  • Explain copays, coinsurance, and deductibles for urgent care visits.
  • Explain basics of our Workers Compensation and Employer Services service lines, along with other ancillary services we may offer.
  • Navigate the practice management system to answer patient billing inquiries.
  • Stays informed of forecasted events that will impact patient volume
  • Receive, route, and convey messages in writing, electronically and verbally.
  • Accept, sort, and distribute Center mail.
  • Scan, file, and fax documents and prepare them for mail-out.
  • Attend and participate in periodic staff meetings.
  • Provide administrative tasks such as responding to emails, making phone calls, typing letters, conducting research, etc.
  • Assist new associates in training as requested.
  • Perform other duties as assigned.

Job Qualifications:


  • Ability to see, hear, and accurately read and write in English.  (Spanish is a plus) 
  • Demonstrate exceptional customer service skills
  • Outstanding verbal and written communication skills.
  • A positive, energetic, and professional demeanor.
  • Demonstrate understanding of when is appropriate to escalate to a Senior vs when to problem solve independently.
  • Demonstrate active listening skills
  • Possess excellent organizational and record keeping skills. 
  • Ability to work independently, prioritize tasks, and meet deadlines. 
  • Proactively identify issues and work to resolve them
  • Demonstrate appropriate discretion in handling confidential and sensitive information
  • Proficiency in Microsoft software products such as Outlook, Word, PowerPoint and Excel.
  • Demonstrate Proficiency with EMR and other ancillary software systems
  • Type 45 WPM
  • Ability to work 8, 10, 13 hour shifts, overtime, on-call, weekends, and holidays as needed


  • Ability to lift 15-30 lbs.
  • Required to move from one location to another within and outside of the building; push, pull and reach; bend; stand for long periods of time; and sit for long periods of time.
  • Must be able to speak and write English fluently.

* 4- 10 hour or 5 - 8 hour days including rotating through holidays and weekends as needed
* Hours: Shifts are flexible
