Description:
Provides spiritual and bereavement support for hospice patients and their family members. The chaplain is a member of the Hospice Interdisciplinary Team (IDT). This position is located in Aurora, IL.

This position requires approximately 10 hours per month.

Primary Functions:
  1. Assessment of spiritual needs of hospice bridge patients and family members.
  2. Participation in development, management and revision of the patient plan of care.
  3. Collaboration with local clergy to provide spiritual support for patient/family, as desired by the patient/family.
  4. Provision of pastoral care for hospice bridge patients/families including religious services of prayer, worship and rituals, as requested.
  5. Respects varying philosophies and religious perspectives. Allows all individuals to discuss and share thoughts, feelings, and beliefs.
  6. Completes accurate and timely documentation for all patient care activities. Follows organization documentation guidelines including electronic and paper processes. Maintains and updates chaplain and bereavement related portions of the patient medical record.  
  7. Attends and participates in interdisciplinary team conferences. Assists team with understanding patient/family spiritual factors. Works with the IDT in a team approach to evaluate patient/family response to the plan of care.
  8. Meets the expressed spiritual needs of patient and family.
  9. Works with other professionals to resolve spiritual, pastoral and ethical issues.
  10. Provides staff orientation and education related to spiritual issues, as requested.
  11. Provides spiritual counseling and bereavement support for individual staff members as needed.
  12. Participates in bereavement support for family members following the death of the patient. Collaborates with other team members to organize and conduct bereavement activities including phone contacts, home visits and support groups.
  13. Participates in community outreach activities such as community educational activities, as requested.
  14. Participates in Quality Improvement activities.
  15. Assumes responsibility for own professional development by attending professional meetings, continuing education and in-services. Completes a minimum of 12 hours continuing education annually.

Compensation: Hourly Range $22.00 to $22.30 per hour, depending on experience.

Available Employee Benefits: 403b (Only eligible if 1,000 or more hours worked per year) 


Qualifications
  1. B.A. or B.S. in religion, theology, or behavioral science. M.Div. or equivalent with ecclesiastical endorsement and board certification preferred. LCSW preferred.
  2. Minimum of four units of clinical pastoral education from a program accredited by the Association of Clinical Pastoral Education preferred.
  3. Interest and previous experience in ministry to families experiencing life-threatening illness preferred.
  4. Maintains current driver’s license and auto insurance.
  5. Maintains current CPR certification.
  6. Health policies and requirements met.

Essential Functions/Physical Requirements:
1. Ability to communicate effectively verbally and in writing.
2. Ability to drive a car.
3. Ability to ambulate in and out of patient’s homes, hospitals, and nursing homes in all weather.
4. Ability to climb stairs.
5. Visual, auditory senses intact.
6. Ability to deal with high level of stress.
7. Ability to wear a particulate respirator during patient care as necessary.
