Family Case Manager(Bilingual Spanish)- Aurora

Description:

Responsible for providing support, education, and linkages to community services for pregnant women, infants, and their families to ensure healthy and safe lifestyles.  Monitors all health visits and documents all activity in the Cornerstone computer system.  Bilingual Spanish required.

 


Primary Functions:
  1. Completes required face-to-face contacts with pregnant women, infants, and their families, either in clinic or in the home, while providing required assessments and education on health and safety issues.
  2. Completes required contacts outside the home with high-risk client population as necessitated by client request or circumstance.
  3. Monitors client activity via WIC group surveys, Cornerstone downloads and/or telephone. 
  4. Develops an Individual Care Plan and provides referrals to the appropriate providers within the community for services as identified in the plan. 
  5. Provides on-going follow up with participants or service providers to determine whether participants have accessed services.
  6. Provides advocacy to assist participants in accessing services.
  7. Maintains professionalism and seeks professional development activities related to FCM.
  8. Submits promptly all time sheets, caseload, and developmental & postpartum depression screening reports.
  9. Works with Family Case Management team to ensure all potential FCM clients are seen during their regular WIC appointments.
  10. Completes developmental and postpartum depression screenings.
  11. Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer (Director of Quality, Education & Risk Management) immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment

Qualifications
  1. One or more years of relevant experience in conjunction with:  Master’s degree or baccalaureate degree in a behavioral science, social science or health-related area, or Licensed Clinical Social Worker, Licensed Registered Nurse, Baccalaureate degree in any other area with one year experience in child, family or community services, Associates degree with two years experience in child, family or community services.
  2. Demonstrates motivation and ability to educate families on health and safety issues.
  3. Non-judgmental.
  4. Culturally competent.
  5. Excellent computer skills required.
  6. Demonstrates proficient writing and organizational skills.
  7. Bi-lingual in Spanish/English as follows: Reading - Required. Written – Preferred. Speaking – Required.     
  8. Valid driver’s license and automobile insurance.

Essential Functions/Physical Requirements:
1. Ability to communicate effectively, both verbally and in writing.
2. Ability to drive a car. (Ability to drive a car entails driving to client’s homes in Kane County area; does not involve transportation of client(s), which is not allowed per program guidelines)
3. Ability to climb stairs and ambulate in and out of clients homes in all weather.
4. Ability to deal with high levels of stress.
5. Able to sit and stand for long periods.
