Intake Coordinator - Community Based Mental Health Programs

Job Responsibilities:

Position Summary: This position is responsible for accepting referrals for admissions into the TCM and CSP programs, conducting the intake interviews, completing intake paperwork, and potentially carrying a small caseload until assigned to a permanent case manager.

Essential Functions:

  • Accept referrals for admission into the TCM/CSP programs; enter all referral information in the computer, review all medical records, and schedule intake appointments for all clients

  • Open TCM/CSP episodes in Provider Connect

  • Conduct intake interviews

  • Place the medical records in appropriate file folders

  • Inform Benefits staff of the new client, what benefits need to be applied for, and input billing information

  • Update spreadsheet to track all new admissions, civil commitments, and sex offender clients

  • Arrange for client transportation to and from WCS at the time of intake

  • Assist with assigning case managers to new clients based on caseload numbers

Other Duties and Responsibilities:

  1. Attend staff meetings as needed

  2. Assist with case management duties when not fully staffed as time allows


Job Qualifications:

Minimum Qualifications:

  • Master’s Degree in a Behavioral Science with documented 1500 hours of experience working with persons with serious and persistent mental illness

  • No convictions or license revocations that would fail a caregiver background check

  • Valid driver’s license with automobile and sufficient insurance to meet agency requirements

Knowledge, Skills and Abilities: Good computer skills including Word and Excel; accurate data entry; good organizational skills and the ability to multitask; ability to meet deadlines; some knowledge of mental illness and the ability to work with individuals diagnosed with mental illness.

Working Conditions: Work is performed primarily in an office environment; most of the work is done sitting at a desk and using the computer; hours average 40 hours per week.

Physical Demands: Work is sedentary in nature and requires sitting for long periods of time with occasional trips to the printer and copier; hand and finger dexterity for data entry.
