Position Summary:

The CLTS Waiver Program is one of Wisconsin’s Home and Community Based Services (HCBS) Medicaid Waiver programs, federally authorized under § 1915(c) of the Social Security Act. HCBS waiver programs provide Medicaid funding that may be used to fund allowable support and services that promote individuals in the waiver access and inclusion in their home and community. Eligible participants include youth between the ages of 0-22 with developmental, severe emotional, and/or physical needs, who are also functionally and financially eligible for Medicaid.

Working under the direction of the CLTS program and Milwaukee County Children Youth and Family Services(CYFS), provide support and training for program staff and work in conjunction with the CLTS Supervisor for the provision of all CLTS services at WCS. This includes providing direct training, coaching, leadership, supervision as needed, and working with the supervisor to provide oversight of a team of approximately 10-16 Service and Support Coordinators.

Essential Duties and Responsibilities

• Be a champion of serving youth and their families experiencing disability needs, carrying a caseload size of approximately 10-15 youth and families, dependent upon program/agency needs.
• Embrace and champion family centered, trauma informed, strengths-based, culturally intelligent services through monthly contact with assigned youth and their guardians, including in-person contact within 10 business days of enrollment, 6-month reviews, annual Functional Screening, and other visits to check in on well-being and review service provision to meet youth/family needs.
• Track due dates and complete Annual Functional Screens to maintain/support assigned youths’ continued eligibility for CLTS services.
• Identify vulnerable and high-risk children and ensure heightened care coordination, collaborating with other agencies as applicable.
• Mandated Reporter responsibilities, including making reports of mental health, safety, or well-being of a child or youth to the Wisconsin Department of Health Services.
• Develop and implement family-centered Individual Service Plans to meet individualized needs of assigned youth and their families and CLTS program requirements.
• Authorizing support and services provided through the CLTS Program, including completion and submitting Prior Authorizations in accordance with program policies and procedures.
• Complete all required consent forms upon enrollment and annually thereafter to comply with HIPAA and programmatic consent requirements.
• Compile and maintain required documentation in county and agency-identified documentation systems, including, but not limited to: progress notes detailing all efforts made for youths and families and all coordination of care to support their needs, Individual Service Plans, Prior Authorization Forms, and any other documents required by Milwaukee County for CLTS programming.
• Provide support, leadership, and role modeling in a strength-based environment to a team of approximately 10-16 Support and Service Coordinators. Provide direct supervision to CLTS Support and Service Coordinators as needed.
• Attend meetings as requested with the Milwaukee County CYFS Management Team.
• Assist Supervisor with tracking systems and follow-up to provide support to Support and Service Coordinators with tracking/meeting deadlines and completing programmatic requirements.
• Track Prior Authorizations in Forward Health for the CLTS Support and Care Coordination Service, editing as needed, to ensure efficient and accurate agency billing.
• Support quality assurance process, including but not limited to qualify assurance calls with CLTS families.
• Complete audits of youth records for the CLTS Team to ensure compliance with state and MetaStar requirements.
• Keep Milwaukee County CYFS informed of emergency or critical incidents and submit the necessary critical incident paperwork.
• Along with supervisor, monitor service utilization, review monthly progress notes, and provide feedback to Support and Service Coordinators and Milwaukee County CYFS as requested.
• Maintain accountability for all Milwaukee County CYFS Policies and Procedures.
• Ensure that accurate family demographic information is maintained by the Support and Service Coordinators and changes reported to Milwaukee County CYFS.
• Provide orientation and training in Support and Service Coordination processes to all new  coordinators, promoting individualized care and use of natural supports.
• Work in conjunction with the supervisor to hold regularly scheduled team meetings to share information regarding Milwaukee County CYFS issues.
• Support team’s established call system from Monday through Friday, 8:00 am to 4:30 pm, to ensure families’ access to Support and Service Coordinators.
• Participate in mandatory trainings and offered in-services and support the team of Support and Service Coordinators in doing the same.
• Provide coverage for Support and Service Coordinators and Supervisor, as needed.
• Maintain internal records related to Milwaukee County CYFS youth and their families using recommended chart format dividers.

Required Qualifications

• Must possess one of the following combinations of degree/experience:
  • Bachelor’s degree in a relevant area of human services, education, sociology, social work, etc. and one year of experience working with specific target group.
  • Bachelor’s degree in a non-relevant area of human services and 4 years of experience working with specific target population.
  • Associate’s degree with minimum of one year of experience working with specific target group.

Knowledge, Skills and Abilities

• Mental Health/Human Development - Working knowledge of mental health needs, cognitive, developmental, and physical disabilities, and positive child/youth development.
• Trauma-Informed Care - Patience and understanding of traumatic life situations.
• De-escalation Skills - interact with escalating youth and/or parents in a calm and professional manner and respond appropriately in crisis situations.

• Technological Aptitude - Must be comfortable with navigating electronic health records, including in Avatar and utilizing Microsoft Word, Excel, PowerPoint, etc. to meet programmatic needs.

• Time Management Skills - Ability to maintain schedules, meet deadlines and multi-task.
• Multicultural Sensitivity - The role involves working in the community and interacting directly with diverse populations, including clients, partners, and service providers. As such, the employee must demonstrate professionalism, cultural sensitivity, and strong interpersonal skills while representing the organization in a variety of settings.

PHYSICAL DEMANDS:

Duties require possible activity while monitoring or interacting with youth in the program. Staff may be required to use physical intervention in extreme situations. Driving required. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

WORK ENVIRONMENT:

Work is performed primarily in a busy office environment managing caseloads of youth, young adults, and families. Some of the work is done sitting at desk using a computer; requires outreach in the community and families’ homes; much of the outreach is done in urban neighborhoods and several hours per day may be spent driving; hours average 40 per week; flexible work schedule include some hours outside the normal work schedule on evenings and weekends.
