Job Summary
Care Coordinators are responsible for providing intensive, individualized care coordination to support individuals—children, adolescents, youth, and adults—through their recovery and wellness journey. They work directly with individuals, families, service providers, and multidisciplinary teams to develop and implement personalized recovery plans tailored to each person’s unique needs.

PHYSICAL DEMANDS:
Position is mobile with time spent in the community, including transporting consumers, home visits, and attending meetings. Driving throughout Milwaukee County. Able to navigate stairs. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.
WORK ENVIRONMENT:
Work is performed in an office and community environment, including people’s homes and various community-based locations. Hours average 40 hours per week. Travel is required. A flexible work schedule is required to meet the needs of staff and the people being served.
